Eligibility Criteria

These are general criteria to be used by the certifying physician or agency representative. In each case, careful evaluation of the applicant’s mobility limitations should be made in determining eligibility.

Under ADA regulations, there are three categories of persons who are eligible for ADA paratransit. Any individual with a disability qualifies who:

is unable, as the result of a physical or mental impairment, to get on, ride, or get off of an accessible vehicle on the public transit system; or needs the assistance of a wheelchair lift or other boarding  assistance device and is able, with such assistance, to get on, ride, and get off of an accessible vehicle, BUT such a vehicle is not available on the route when the individual wants to travel; or has a specific impairment-related condition (including vision, hearing or impairments causing disorientation), which prevents travel to or from a station or stop on the system.

What if I do not qualify for the LIFT?

Establishing LIFT eligibility

To establish eligibility two forms must be completed and mailed to the LIFT. The necessary forms to apply are included in the file below. Please note the Doctor Questionnaire portion must be filled out by your doctor:

Each application will receive a prompt review. Eligible applicants can get a numbered picture identification card free of charge at the

Lost cards can be replaced for $5.

When eligibility questions arise, an advisory committee will make the final determination with assistance from medical personnel when necessary.

If, after reviewing the eligibility requirements, you feel you may not qualify for the LIFT, information for using Sun Metro's accessible fixed routes can be obtained by calling the Sun Metro information services line at 915.533.3333.
